Generally- I do some degree of capsulectomy at the time of implant removal to facilitate healing of the overlying tissue ( muscle or gland or both ) and avoid chronic seroma. In may cases- especially with capsular contracture or rupture- total capsulectomy is necessary.

Swelling in the feet and legs can certainly occur after such an extensive procedure. The most critical thing to rule out is deep venous thrombosis which can occur after abdominoplasty and can be life threatening. I would get evaluation from your surgeon and perhaps get Duplex studies of your deep leg veins if indicated.
Tummy tucks do interrupt the normal lymphatic flow and compression garments sometime control post op swelling- but not in your feet. It may be related to that.

I have seen this in a few patients and with certain non absorbable braided sutures. They can get chronically colonized with bacteria and have late infection . I have seen sutures “spit” out through the skin or belly button years later with one particular type of suture. Especially if your original surgery might have had some wound healing problems. Hard to diagnose this for sure. CT scan might not show much. Sometimes limited exploration to determine if this is going on is necessary….

Silicone gel sheeting may help although it is best used early in the post op period. Laser treatments are also an option. If your operating surgeon doesn’t offer help, seek a Board Certified plastic surgeon for help in managing your scars. Improvements are definitely possible

Progressive tension sutures

One of the potential problems of drainless tummy tucks is the progressive tension sutures which fix the elevated abdominal wall back down to the abdominal fascia. If overly aggressive- they can scar down the skin to the midline and create a line . Such tethering may need to be surgically released if it doesn’t relax with massage.
It usually doesn’t become apparent til the swelling goes down which may be months after surgery.

Unfortunately, I don’t think there is an absolute right answer. In general, sub muscular positioning affords a better chance of preventing recurrent cap con so option two probably safer and more predictable although it involves two procedures.

It is still way too early to judge results of your reduction. It takes many weeks even months for things to settle down. Even if there was a revision anticipated- most surgeons wouldn’t do anything for 6-9 months. The result ” on the table” meaning during surgery almost never resembles the final result.

With proper documentation of neck pain and back pain, etc you probably would qualify. Selection of size has to be determined with your plastic surgeon. The smaller you go, the more chance for loss of sensation and potentially sacrifice shape. Going from H to A is pretty radical, possible , yes but might not be advisable.

Repair of an implant related problem with mastopexy can be a challenge. I suspect removal will leave you more saggy. Hard to tell without a photo but these problems are correctable. Sometimes removing the implant for a while to allow “settling” is advisable and gives you a chance to live with that situation for a while before deciding if you want replacement

Pain that remote is a concern. If it is in your axilla( armpit) it could mean late recurrence. If related to an implant reconstruction- it may be related to a capsule. Best advice is to see your oncologist who should be able to help find the source

I would listen to your PS recommendations. You are risking disrupting all of the work put into trying to achieve a good result. Especially with a direct to implant procedure which is asking more of your tissues and muscle than an expander result.Did you expect someone to say “Go ahead- ignore your surgeon’s recommendations”?

Impossible to say- but generally the larger the implant size – the more the breast will sag. It seems awfully early to be removing implants( 2 years later)- they look fine in these selfies but only your original surgeon can tell. If you insist on removing them. I would wait before considering additional surgery until whatever caused you to be unhappy with them has resolved and your breasts have had time to heal.

Looks like you have some necrosis on the right. Not uncommon with a diabetic. I agree with good wound care. Hopefully things will heal but you might need a revision after full recovery. Your doctor is being very diligent about seeing you every week.

I think you will probably need some skin removal in the future. Your preop picture revealed a lot of skin excess and I usually offer a mastectomy or breast reduction technique in this instance. However, I would follow closely with your doctor and evaluate further after more time has passed

I am reluctant to recommend anything less than a week off for a breast reduction for patients with desk jobs. Everyone is different and every situation varies but as a general rule- I have found a week to 10 days is desired. Many patients are still taking pain medication for up to week. I think most physicians are not willing to have patients disappointed and unhappy if they say you can go back earlier and then you have to miss work and leave your employer…
