Would you recommend I have a breast lift or can I have just a breast augmentation?

The answer of course is “it depends”. For a smaller augmentation, the skin won’t fill out enough to avoid sagging of the breasts off the implants. However, larger implants certainly may fill out the breasts and avoid a lift. Measurements, skin elasticity, and density of the breast tissue all affect the nature of an augmentation and require in person evaluation.

Initially, you will need to get closer to your goal weight of 150lbs. This will affect the size of your breasts and may also affect your goals from the surgery. Fat grafting works well in small volumes, but is usually unrealistic for a large size increase. Implants will reliably give you larger breasts, but you will definitely require a breastlift to position the breast tissue and the nipples in the correct place.

The breast tissue is being pushed forward by the implant, however there are restrictive tissue bands inside the breast which are contributing to the distortion. These can be released surgically to allow the breast tissue to properly redrape over the implant.

Constricted breast deformity

Tuberous breasts are a variant of the common “constricted breast” deformity. This is a congenital deformity where the inframammary folds are elevated and the breasts hang low. Rounder breasts require a mastopexy and sometimes benefit from implants. The fold underneath the breast may need to be lowered. In your case, I don’t believe you have a traditional “tuberous” breast, but you certainly appear to have a constricted breast deformity.

The uniboob deformity arises from communication of the implant spaces or violation of the sternal skin. Correction is a difficult procedure and requires a new pocket, repair of the existing communication, and sometimes dermal matrix for reinforcement. This is essentially a reconstruction despite being considered cosmetic by insurance companies. A particularly useful approach is the neo-subpectoral pocket approach we designed at Georgetown many years ago.
Should I go for 365cc, 385cc, or 415cc? Currently 5’5 105lbs.

The most important factor in implant sizing is the base width of the implant. The implant should be narrower than your breast width, otherwise the implants will push to the side giving you a heavy, even look. 415cc is a large implant, and will look very big on your smaller frame. Your goals in regards to fullness, “natural vs. fake”, and projection all play into the implant choice. If you are confused, second opinion consults are a good way to sort out the issues.

When you have minimal breast tissue, your breast will take on the shape of the implant placed. Shaped implants can help, but they are firmer and don’t move due to the texturing. There is also the concern of increasing risk of lymphoma. Moderate Plus style implants or Moderate Plus Xtra implants do a great job of maintaining a more “natural” appearance while providing volume. An increase from 470-590cc will certainly be noticeable. For your size, this is about half a cup size. Be aware that going too large can lead to long term complications such has overly stretching the skin, bottoming out of the implants, and significant shape changes with menopause.

A change in cup size is usually between 250-350cc depending on overall anatomy. 750cc are extremely large implants and would likely put you in a D/DD range. I would recommend consulting with additional plastic surgeons and you will find that implants in the 300cc range are more likely to give you what you want.

Donut mastopexies involve a lot of tension around the areola. When large implants are placed, they can exhibit a lot of additional pressure on the skin. This combination is not always ideal and can lead to very wide areolas and poor results.

You do not appear to have a tuberous breast deformity, however you have a mild constriction of the right lower breast pole. This can be improved by releasing the constricting bands through an areolar incision when placing the implants. The symmetry can be improved, but it may take several months before you see the final results.

The reason to go underneath the muscle in a breast augmentation is for camouflage of the implants. There are side benefits as well – a smoother upper pole transition from chest wall to breast and a lower capsular contracture rate. However, there are a few negatives as well. You should have this evaluated by a Plastic Surgeon in your area. There are several reasons why this can occur but the bottom line is that imaging and a physical examination are required. It certainly may require surgery.

Capsular contracture is diagnosed on exam as the firmness of the capsule is significant. An elevated implant can simply be from improper pocket placement (common in umbilical placements). It is unlikely that your implants are smooth if there are teardrop as all anatomic implants are textured. If you choose to explant, it is recommended that you remove the capsules as well.
