Hello,You should consider confirming that you have a rupture via either an ultrasound (least invasive) or an MRI (most specific to detect ruptures) and that may help guide your decision about timing. I typically wait a minimum of 6 weeks after breastfeeding to explant depending on severity of pain/symptoms otherwise 3 months is ideal to allow the breast tissue to settle. Good questions! Given your history of multiple breast surgeries related to implant issues I would suggest doing an Explant with Total Capsulectomy along with a breast lift. The lift will address your loose skin and inframammary fold issues. The tenting is a result of the implants pulling on the skin/tissue from the symmastia- which will get better by removing the implants. Good Question. While most patients opt to remove the capsule and implant (which is my personal preference) , yes you can do fat transfer after implant removal only while leaving the capsule behind. You will want to wait 6 months to a year for the skin to ‘shrink’ so you can get an accurate fat transfer for optimal cosmetic outcome. Simple Implant removal can be done under local anesthesia.

Based on your current BMI, I suggest losing weight prior to any surgical intervention. Specially if you are considering a BBL- if you loose weight after the surgery, that equates to loss of volume in the buttocks as well.
The buttocks will be hard initially after surgery and will soften up over the course of a year.

You should wait anywhere from 3-6 month after breastfeeding or pumping and dumping prior to doing any surgery. This allows time for your breasts to settle and not be engorged or swollen. Insurance will not cover the cosmetic portion of your procedure. I do not think you should combine the hemorrhoid procedure with the mommy makeover. Also, depending on the size of the hernia- mesh repair is not always required and this should be reviewed based on physical exam.

Having completed training in general surgery I can appreciate your concerns. Tummy Tuck procedures do not involve any manipulation of the bowel- being that it is performed in a superificial layer above the abdominal fascia. As with all surgeries there is a risk of narcotic or anesthesia induced constipation or decreased gut motility. These may mimic or exacerbate a SBO.
