If you are not opposed to having breast implants I would recommend for you to have a breast lift with implants after your 40 LBS weight lose to get the best shape and rejuvenate your breast. I think you would be a good candidate for a lower body contouring procedure such as a tummy tuck or belt lipectomy (lower body lift). As for the breasts, you would benefit from a lift (mastopexy) and possibly placement of an implant. With massive weight loss patients, since the skin has showed such a tendency to stretch, I often recommend to do the lift first, then place the implant at a second operation. This allows the skin to heal first and also offers you the chance to better determine what size you want to be in terms of implants.

I would say that if your overall health is good, you could consider having the tummy tuck and lift first, and then go back and place the implants at a second operation 3-6 months later. Or have the lift first, then go back and place implants at the time of the tummy tuck. Either would be a fine option. Make sure you have several consultations with board certified (ABPS) plastic surgeons in your area before you choose what approach would be best for you.
