Saggy skin with tissue expanders after nipple-sparing mastectomy?

You have a lot of excess skin that should have been adjusted at the time of the mastectomy. The expanded width and projection is not enough to stretch out the skin. Over filling the expander will not help as the device will become narrower.At the second stage a wider more projecting implant can be used along with a mastopexy to remove any lose skin. This should be only around the areolae which are too wide. This is readily fixable!

It is still early in your post operative course and you should wait almost a year to allow all tissues to heal and the ” final ” result determined. At that time you and your surgeon can evaluate and discuss options. Removing residual breast tissue, excess skin is relatively east. The nipple positions should be more appropriate after that. Minor adjustments may be suggested.

Without photos, it is difficult to make recommendations. The history of two surgeries with lifts, and now suggesting removing implants is concerning. I believe another opinion with a surgeon well experienced in revision all breast surely is warranted. A change in implant location is a possible option instead of removal.

Medical tourism can be dangerous as surgeon and facility certification may be substandard. Make sure the surgeon is a member of the American Society of Plastic Surgeons, which does accept international members. This will assure a level of competence. You can have all you are requesting done, but maybe not in one surgery. I respectfully disagree with one of the answers about a TRAM flap. You do not have any fatty tissue, in your abdomen to rebuild your breasts. An Abdominoplasty with a reconstructive repair can be performed in conjunction with a robotic or laparoscopic oophorectomy ( ovaries removed). To obtain a nice result, you should lose 10-15 pounds and then have an Abdominoplasty with possible liposuction of the sides. Liposuction alone is not appropriate and will not attain good results.

Smoking increases the risk of complications associated with surgery especially when skin flaps are raised. The micocirculation of skin is decreased as a result of the compounds in smoke. This increases wound healing problems, skin dying ( necrosis), higher infection rates, and anesthetic complications. You need to stop smoking several months before surgery. Your surgeon can check your urine right before surgery to make sure you have really stopped!

Petite, athletic women, like yourself have to careful of making your breasts too large. Heavy devices can stretch the breast tissue causing bottoming out. You already have this on your right side. Don’t focus on volume, speak with your surgeon on the dimensions you are looking for. A larger device can be placed along with revision of your right fold position.

No surgeon should promise the preservation of nipple sensation following breast surgery even if liposuction is the only approach used. You have to be willing to possibly diminish or lose sensation with Top surgery.

It is still early and the final result has not presented itself. You should wait at least 6-9 months to make a determination of how your chest looks. Then a revision can be performed if needed. Most likely a skin eduction and placing the lower incisions at an even level, removed excess skin, relocate the nipples and reduce their size. We do not see any pre operative photos so we cannot criticize your surgeon. These sometimes have to be stage procedures.
