Yours is a very unpredictable surgery regarding the results. Without going into many details, and not seeing your preop pics, I think you may very satisfied with revision surgery.
Money and healing time are a major concern, but you may consider talking with your surgeon about what expectations of revision may be.

It seems you left breast may have a capsular contracture. You right breast actually moves in a natural, expected way. Your plastic surgeon may consider tightening the capsule on the right keep it more toward the center. Your left breast may need some scar tissue removed. Scarring as you have can be very difficult to improve. There are several approaches to this. You could review these options with your plastic surgeon.

From your photographs, it seems you would not be a good candidate for just breast augmentation. Your nipples would remain in a very low position. I think a breast lift would give you a very nice result. Whether you want and augmentation or not, as to be determined by you and your doctor.

To make this decision, I would need more information than your picture. There are many considerations when we decide who is a candidate for a tummy tuck. It seems you have quite a bit of extra fat, and may be a candidate for liposuction followed by a tummy tuck a number of months later.

I would say that your first step would be to get a good exam by a plastic surgeon. You would probably be able to get a very direct answer about exactly how much of the stretch marks would be removed. This would be determined entirely on how much skin is excised. This can often be determined quite well preoperatively.

I have never heard of loosening of the vaginal wall with this type of surgery. Perhaps if you mean the labia majora, this definitely could happen if that area is not manipulated well during surgery.

From your photographs, it is impossible to know the cause of your bulge. A good examination would be very revealing. The easiest possibility would be that the bold is largely from extra fat, and this could be remedied very easily with liposuction. It might not correct completely, but perhaps enough to be acceptable. It may be helpful to seek a second opinion.

Looking for advice on revision. Will replacing silicone with saline help with how they sit?

I don't think that switching to saline implants would change the way they sit on your chest wall at all. From your photographs, it seems you do not need a lift, and it also seems that changing the pocket will not change the overall look very much. Sometimes the number of surgical options to make these kind of changes are quite limited.

From your photographs, there seems to be no reason why cannot get bigger implants which are positioned lower to rotate your nipples forward. You could get this surgery at any point. You might want to start with your plastic surgeon to see how they would approach this. If this does not meet your expectations, you may want another opinion.

You could achieve a D cup without using a high profile implant. Chasing small diameter differences to conform to a chart usually isn't very helpful. It is best to sit down with a plastic surgeon and discuss what very subtle differences you may get with the various implants and what will approach your expectations the closest.

As long as you are healthy, your BMI is quite normal and would not exclude you from being a candidate for a tummy tuck. We generally think in terms of a BMI of 30 as getting high. It may be safe to do a tummy tuck on a large part of the population who have higher BMIs than this, if extra attention is paid to a number of factors. Sometimes the procedure itself can be adapted to such patients.

I think the best strategy is to consider there before and after photographs and just go with the doctor you think best understands your expectations.

I recently read that pierced nipples prior to BA can cause CC. Any truth to this?

Pierced nipples will not cause capsular contracture. The main concerns is cleaning the area well to assure it is sterile. I have this done in the operating room and then I cover them with and occlusive dressing. The rings can be replaced in the operating room by the staff or later by you.
