One answer you will always get here because it is a good answer is, "go talk to your surgeon." The pouch that I make out from the photos is one that may be because of difference in tissues before surgery, extra swelling during surgery that make completely perfect results impossible, or some fluid that collected after surgery. These are all things that happen at times even with the surgeon doing everything properly. Also it probably wouldn't take a very big procedure...

You should definitely work out your schedule with his or hers to get in. Often seeing the doctor and staff will help you understand the healing process and can lift a bit of those blues. It may be that the doctor had to cancel that appointment in order to take care of someone with more pressing needs and probably wants to take just as good of care of you. What you want can be done. The main issue as others point out is that the scar could be the issue. I say could because that all depends upon you and how much the scar would bother you. Usually scars are thin and pale when finally healed, but unfortunately there is no guarantee on that and even at their best are always visible to some degree. If you are absolutely sure that the scar would not be an issue, even if noticeable, then it is something you could consider.

The main answer you always will get here (because it is a good one) is to make sure you see your surgeon and get an exam in person. Implants generally do fall at different rates and it can be for a couple of months for them to get more even. You do not look notably different from routine healing in that respect. You might find some surgeons who would charge more for going extra large and that may not be unreasonable given all of the extra risk that go with the surgery and increased chances of future surgery for complications. In our practice we don't charge differently for size, but because different implants have different cost, that will affect price. From the pictures it appears that your muscles have been stretched so far apart by the pregnancies that you would benefit from having them repaired with the tummy tuck. If you do skin only, likely you would still have a bulge you would not be happy with. See your local board certified plastic surgeon so you can have them evaluate you in person and give you all the answer that you look for.

I think the best you can do to communicate with your surgeon is just like you have here, with photos of breast you like. From my perspective, the pictures you are probably someone with a lot more of their own breast tissue to begin with. The same size/style implant on your frame would likely provide a much different result.You may be looking for implants that exceed your diameter. Having a little "side boob" on you might not be bad since your areola are a bit outisde the midline...
