Thanks for this question.  The “en bloc” removal of implants refers to the removal of the capsule and implants in one piece without violating the capsule.  The factors that will determine if this is possible are the amount of remaining breast tissue as well as the size of the revision incision.  The most important factor is meticulous removal but not at the expense of sacrificing too much breast tissue.

Thanks for your question.  This is a difficult case without photos and an exam.  If your breasts have become stretched out from nursing and there is redundant tissue, I recommend a mastopexy coupled with fat transfer to enhance the upper pole and medial breast fullness.  Please go see a board certified plastic surgeon who is experienced with all facets of breast surgery to hear all of your options.  Please also share your results.Best Wishes

Thanks for enclosing your photos.  Fat transfer for the breasts will give you some more fullness.  It does not take the place of an implant, but it will allow you to go up about one cup size.  You have adequate stores of fat in your flanks, lower abdomen, arms, and medial and lateral thighs.  The overall result will be some increased definition of your waist and more sculpted arms coupled with an increase in the fullness of your breasts.

Thanks for enclosing a photo.  Voluma is a great way to add definition to your cheeks and to do so in a subtle fashion.  I agree with many of these posts in that 0.5cc seems small, however, the benefit is that if you like the result after the first syringe you can go and have more injected.  Keep us posted.

Thanks for the pictures.  Liposuction is a great adjunct to reducing the appearance of the back rolls.  Depending on the severity of this area in terms of the amount of fat and the degree of adherence will determine what the best course of action is.  If you had a back lift arm lift and tummy tuck, I would gather that you did not have a significant liposuction at that time of that procedure.  You could definitely benefit from liposuction to decrease these areas of concern.

As plastic surgeons we are trained to the “ideal” however, the reality is that your opinion is the one that matters the most. The other option that helps is to perform a composite breast augmentation and add fat transfer to help support the soft tissue of your breast. Thanks for bringing up a great question. There are many ways to get training in cosmetic surgery. Some of those involve carefully designed residencies and fellowships that are accredited by Licensed and recognized boards (American Board of Medical Specialists) and then there are those that aren’t.) As an RN, you should have further insight into the best practitioners in your community. Please go see a physician who is board certified in plastic surgery.
