After accepting that I may not receive the results I indeed wanted, what can I realistically expec
Realistic expectations ??
May 11, 2017
First of all, thank you for your question. I do not believe that your desired result is achievable with your present anatomy.  You show a photo of a primary augmentation, where your anatomy dictates some type of breast lift or at least control of your large areolar diameter.  You have a variation of a tuberous breast deformity, best treated with a silicone implant, rather that saline, as well as a procedure to control further widening of the areolae. This requires at least a periareolar ma…
Is there a chance I could get a reasonable result with just implants and no lift?
Implants without breast lift
May 11, 2017
Thank you for your question. This is a very common question and very common concern.  Virtually, no one wants the incisions associated with a mastopexy. I doubt very much that you would be content with a primary augmentation as the implants would be low, matronly and give you an aged appearance.  The chance of a double bubble could be lessened with a subglandular approach, but compromising now will invariably lead you to significant surgical revisions down the road if you don’t consider a b…
Is ‘gummy bear’ my best choice?
Gummy Bear Implants
May 11, 2017
First of all, very difficult to provide recommendations for size without photos and physical assessment. Infact, I do not ever speak of Triple DD bra sizes because I really have no idea what that means.  Neither does any one else, I believe.Gummy bear implants are shaped textured devices that in my opinion have limited role in primary breast augmentation.  Firstly the are firm, with prominent edge palpability and often visibility, have the ability to rotate despite their textured surfaces, a…

size concerns
Apr 25, 2017
Thank you for your question.  I think you should stop torturing yourself over implant differences of 6 to 21 cc’s.  The differences in terms of volume are quite minimal.  I recommend using a gel sizing system wearing clothing so that you see what your result will look like since I assume that you are clothed most of the day.  Additionally, discussions of specific bra sizes are very difficult based upon different manufacturers. Lastly, go into your kitchen and measure out 15cc of fluid to …
How old is too old for a breast lift? 53 yrs, 5’5, 135 lbs w dense breasts. Good candidate or not?
Too old?
Apr 25, 2017
53 is a very normal age to consider these  types of procedures. Breast lifting with a small reduction has a high level of patient satisfaction.  It is usually an easy recovery with a rapid return to normal functions.  Brachioplasty on the other hand probably yields the worst looking scars in all of plastic surgery. It is a very effective procedure for removing excess skin at the expense of a rather obvious scar. Long term scarring can be quite variable however.   I show my patients photos f…
Issues with donut lift?
periareolar mastopexy
Apr 25, 2017
Thank you for your question.  Unfortunately pre op photos would have been very helpful here.  Based upon your photograph, it looks as though your surgery was relatively recent. Yes, implants appear high now, but you need to give this some time to settle.  Too early to decide whether or not a breast lift is necessary, but if so, I believe it would be done with a vertical technique, not the full anchor procedure.
Should I get a fat transfer or implants?
Implants versus fat grafting-
Apr 25, 2017
No question that breast augmentation with implants is more predictable and reproducible.  Fat grafting can be a bit finicky as resorption rates can vary widely.  My main concern is your disclosure of not having much fat for donor site.  In general, silicone works much better in thinner patients. The number of cc’s would require a face to face consultation not only for examination, but a conversation as to what your goals and desires are.  Best of luck!
Will 375cc HP give me natural full boobs? Or should I do 375 moderate plus?
breast implant projections
Apr 25, 2017
Thank you for your question. In general, assessment of soft tissue goes hand in hand with measurements.  11 cm likely refers to your current breast diameter and at a volume of 375cc is actually closer to an ultra high profile implant.  That is by measurements alone.  I use the Mentor gel sizing system and have been happy with that.  Talk to your doctor and express your concerns.  My opinion is that ultra high will look a bit fake and that moderate profile plus may be a bit too wide for you a…
Facelift Recovery
facelift downtime
Apr 23, 2017
No solid answer here.  Most will say 2 to 3 weeks.  Could be one week, could be 4.  In general facelifts alone heal a bit quicker than when combined with eyelid surgery. Bruising from eyelid surgery is often much more obvious.  Secondly, a person’s willingness to go out with some bruising or swelling is also critical in this discussion.  Just don’t plan any major events immediately after surgery……
I am a month and a half post face lift and rhinoplasty. Will oral steroids help reduce swelling?
steroids for post op swelling
Apr 23, 2017
Thank you for your question.  In general, if one believes in steroids for post op swelling, they are either given at the time of the procedure or immediately post op.  Swelling at 6 weeks can still be present and even significant.  Steroids at this point would be an unusual course of treatment. Bring your concerns up with your surgeon and have patience as this will take some time.
High profile silicone 450 or 485 ccs for someone who Lost weight but still has about a B cup? Want
35 cc difference. What do I do?
Apr 23, 2017
Thank you for your questions, but unfortunately trying to decide between two large implants that vary by approximately an ounce and discussing bra sizes, which are notoriously inconsistent depending on manufacturers, without photos to assess is absolutely impossible.  Don’t fret about this rather insignificant difference.  More importantly, talk to your surgeon, utilize sizers to see what you will look like and be specific. No one on line will be able to give specific advice, especially w…
In treating breast asymmetry, which is more invasive, a breast reduction or a breast implant?
options for breast asymmetry correction
Apr 23, 2017
This is a complex question, with a number of options and many issues related to complexity and potential surgical complications. I personally ask my patients the simple question: do you like the size of the larger breast or the smaller breast better?.  If they like the smaller breast better, then reduction with or without a breast lift on the either side is the clear option.  If they like the size of the larger breast better, then options are numerous and really beyond the scope of an i…
Are my implants too far apart? What can I do?
Separation of breast implants
Apr 23, 2017
In general, patients vary in terms of their anatomy, so numerous factors must be considered when choosing implants, pocket selection, etc.  Most of the time, submuscular implants tend to be wider than subglandular devices given the same implant.  Selection of implant volume, projection and width are critical to obtain the best result.  Talk to your plastic surgeon, or contact an experienced board certified plastic surgeon for a consultation to see whether any options are available to you to…
Is the anchor lift technique really needed?
Full anchor lift necessary?
Aug 12, 2015
Not certain about full anchor lift, but definitely would require some horizontal incision in the breast fold. This area can be “cheated” to minimize the length of the incision. Doubt that you would get an acceptable result with no incision in the breast fold. Best of luck.
Will this fix itself?
Breast asymmetry
Aug 12, 2015
I agree with you as well as other comments that you are very early in the post op course. Preop your Right breast falls to the side of the chest wall and appears a bit lower. Your left chest wall seems to be a bit more projecting. Slight asymmetry is often seen although not always welcomed. Give this time ( 3 to 6 months) before you get too concerned. Best of luck.
