Doctor’s answers

Question
Answer Header & Date
Answer Snippet
Two previous breast lifts done – would a third lift be too risky?
Consultation
Mar 24, 2023
Thank you for your question. This is something we see frequently in our practice. Scar tissue will never be as strong as native/uncut tissue. For this reason when we perform secondary, tertiary, etc breast surgery we encourage patients to use the internal mesh, that is dissolvable over time, but initially helps to strengthen the support of the tissue to hold the implant up. Sometimes if the position of the previously lifted nipple is in the correction position but the bottom is “bottomed…
Age 36, do I have stage 1 ptosis & is BL & augmentation required?
Expectations
Mar 24, 2023
The answer to this question really depends on what you’re looking for in the overall appearance of your breast AFTER the surgery. If you are looking to keep it small/conservative but perky – nothing more than a moderate profile implant than more than likely you could benefit from a type of lift. However, you would also be a good candidate for the dual-plane breast augmentation using a full profile (higher volume for your breast width) implant with possible placement of internal bra to…
Help with size of implants? 37, 5’4, 137lbs, recommended 500/550cc, behind muscle
The Breast “Footprint”
Mar 24, 2023
When discussing the implant volume that fits you best, it is essential to base it on the geometry of your breast width. When patients come to me, I will typically measure the width of their breast in centimeters to determine the width, or diameter, of the implant that should be used to fit your body. Once we narrow that down to within 0.5-0.75 cm we look at what profile of breast the patient is interested in having to achieve a certain level of upper pole fullness. The profile choice and the…
I’m a small 34B, 4’11 and I want to be at least a 34DD. Would 695cc implants be possible for my frame?
Volume
Nov 25, 2020
Thank you for this question. The amount of volume that can be places submuscular is determine by the width of your frame/muscle/breast tissue. So, being petite it may be impossible in one stage to fit that amount of volume without expanding the pocket with a smaller implant first.
We hope this helps!
Help me figure out if I should go for 300cc or 330cc
Volume vs Profile
Nov 25, 2020
Thank you for this question. We get this alot in our practice. When people question volume we ask them about what they are looking for a “natural” shape? A “little more than natural” shape? Or a “wow those are fake!” Shape.
This helps us determine the profile of implant you get. If you are set on a moderate profile for the more natural shape it will not make a difference other than in Width of your breast. The projection of 30cc is non-existant. In other words a moderare profile implant to …
What would the difference be between Mentor Silicone 450cc Moderate Plus and Mentor Silicone 500cc High
Profile
Nov 25, 2020
Thank you for this question. You bring up a strong point about profile. In general a higher profile implant will be more dramatic of a change with more upper pole fullness than a moderate plus. The 50cc isn’t really such a big difference as it is basically 1.5x medicine cups of fluid. And the 0.4cm of width will not create much more side boob. Hope this helps!
What size of implant and profile for petite female?
Volume and profile
Nov 25, 2020
Thank you for posting this question. Women with such a petite body size and breast shape may need sometimes two breast augs to stretch their skin. If your breast diameter is 11.5cm you really cant put anything wider than 10.5cm in a high profile for max volume. Most likely a high 270-295cc. Thats really stretching it. The general rule is for every 150cc you go up is 1 cup size. I’m sure someone may measure you as a c cup afterwards but maybe not all the time. Hope this helps.
I’m 5’7 135 pounds and am 20 years old. 350 or 400 cc implants?
Profile vs Volume
Nov 25, 2020
Thank you for this question. I find that alot of potential patients are misguided with implants in 50cc increments.
The more modern way of doing this is to use your own breast width to set the diameter of the implant and then if cleavage is your main issue talk about the PROFILE of the implant. Most of the time we ask, “Would you like to look natural, a little more than natural, or very augmented”. This sets up the profile being moderate, full, pr high profile. That will give you the VOL…
Tuberous breast augmentation without a lift?
Tuber Breast
Nov 25, 2020
Thank you for your question. The tuberous breast correction really depends on the grade of tuberous breast. In your case you have a high riding Inframammary Fold and Nipple Herniatio of tissue. For this case you may not need an entire life with vertical scarring. We would typically perform a dual-plane aug with the incision lowering the fold and a periareolar reduction scar to reduce the herniated tissue and cone shape. With some radial scoring it will help round out the tissues.
Is 500cc too large for me? I am 5 foot 9, 148 pounds with an average frame.
Custom Fit
Nov 25, 2020
Typically your surgeon should be able to tell you what size will fit within your body by measuring you breast width. There are 600 different sizes of inplants with a different size every 15cc and 5 different profiles for each. Typically if your breast measured 14cm wide you could use a 13cm wide implant and the most volume would be a high profile. This would be 500+ cc…however if, judging by your picture, you measure around 12.5-13cm breast width we could only use a 11.5-12cm diameter…

Dual Plane
Nov 25, 2020
Hello and thank you for your question. I see alot of women with your breast shape. You have more skin than breast tissue and need to “reinflate”. In this scenario we perform a dual-plane breast augmentation which places the implant partially submuscular and partially subglandular for a more natrually full shape.
Does laying on your side at night after a breast augmentation cause soreness?
Side Sleepers and Breast Augmentation

Thank you for this question. Typically after your breast procedure you should not be lying on your side for a timeframe usually determined by your plastic surgeon. The ideal position for the first 3 months after the procedure is on your back with a sports bra on at all times. You do not want your capsule/pocket to form to far laterally (armpit) or medially (causing symmastia) which can occur when placing pressure on one side by lying on your side.
I was told that you can’t use inner thighs to lipo for BBL and that it had to be a separate procedure.
Limitations

Thank you for asking this question. Sometimes we are limited to the amount of fat we are allowed to take in total by state regulations for your safety. It is up to the surgeon’s discretion where we take the fat and using his/her better judgement if they will reach these limits with say, just abdomen/flanks or abdomen/flanks/back. You don’t want to start a new area if you cannot complete it within the limitations, because it will just scar in the area and make it more dif…
Large implant alone or lift and implant?
Necessary Lift
Jan 22, 2018
This a a question we face every day. However, the answer is typically the same. We cannot place an implant under the muscle and not do a lift in ptotic breasts. It can cause a deformity called “double bubble”. If you wish to be perky and have youthful cleavage like the wish pictures, you will need a lift. Simply placing large implants subglandular in ptotic breasts will just make them sag more along with your cleavage. I hope this helps your decision making.
Is 34 units of Botox too much?
Botox Treatment

Thank you for the question. 34 units is typically not out of the normal threshold to encompass the 11s and the crows feet. Some people require 20-30 units just near the 11s (Glabellar area). Each eye can require 6-12 units for the crows feet depending on the patient. It is all based on a case by case dosage requirement to relax the muscle. The muscles will not atrophy away, but they will get weaker over time as you repeatedly inhibit them with the neurotoxin. You…
