I close the small incisions after liposuction. Closing the incisions allow them to heal better and will look better as they heal. I do try to remove as much of the fluid as possible so there is less drainage. Your body will absorb the remaining fluid and your kidneys will process the extra fluid.

It is possible to reduce the size of the areolas with surgery. The outside part can be resected and the areola sized smaller. Make an appointment with board certified plastic surgeon who can advise you. Best wishes.

As part of your consultation, discuss with your plastic surgeon your concerns and goals regarding nipple placement and size.  It would be helpful if you bring some photos of your ideal placement and size of nipples.  Typically, the areolas are made smaller and higher during a lift and asymmetry is addressed also.   Make sure your plastic surgeon understands your concerns and goals for surgery. Double lumen implants have an outer layer of saline that surrounds the inner implant of silicone.  It is highly likely in 34 year old implants that the outer shell that contains the saline has deflated.  The inner shell containing the silicone also has a high likelihood of leaking.  I would recommend you have the implants and the capsules removed.  If you wish, you could leave the implants out or replace the implants with either saline or silicone implants. Whether you wish to have your implants removed under local or general anesthetic, your hematologist, in conjunction with your plastic surgeon, will give you recommendations for your particular situation.  If you need your capsules out due to ruptured implants or other reasons, you will need general anesthetic.  Best wishes.

With ruptured implants, it is best to have the implants removed along with the surrounding capsule.  You may wish to leave out the implants or you could replace them.  The implant manufacturer will provide implants, for ruptured implants, should you wish to replace them.  Your warranty for additional monetary assistance from the manufacturer ends at 10 years.  Consult with plastic surgeons who can give you specific recommendations for surgery for you.  Best wishes.

Once the implants (and part or all of the capsules)  are removed, the breast tissue settles and adheres or grows back together.  The air that was in the pocket is removed by surgical drains, and not absorbed into the vascular system (an embolism).

It is possible to remove your current implants and replace them with smaller ones and have a breast lift at the same time.  The condition of your skin/elasticity and blood supply to the nipples must be taken into account.  Do to your large implants, you may need some revision of the pocket to accommodate smaller implants.  Consult with your plastic surgeon to discuss your surgical options.  Best wishes.

You may want removal of your implants and capsule in one piece, en bloc, especially if your implant is leaking or ruptured.  Some patients wish to have en bloc removal due to  medical conditions arising from their implants.  Other patients want a complete capsulectomy to remove all of the thickened capsule. It is possible to do a breast lift at the time of your explantation. Discuss with your plastic surgeon your concerns and goals for surgery.  Best wishes.

A thickened capsule from a grade III encapsulation is best removed.  If the thickened capsule is left after implant removal, the capsule left behind may leave a visual deformity or palpable mass.  The retained capsule may increase the likelihood of fluid buildup-seroma.  Very little breast tissue if any will be removed with removal of the capsule.  Hope this helps. From your history, it seems that the pocket is larger on one side and the implant then falls to to the side.  The pocket does not need to be stitched down if you are removing your implants.   With removal of  implants, this problem should be gone.  Usually the capsule is very thin and flimsy with saline implants and does not require removal unless you want it removed.   Drains are commonly used in this surgery to help decrease the chance of a seroma.  Discuss all of your concerns with yo...

It appears from your history that your saline capsule is soft and not hard or thickened.  Unless you are experiencing some symptoms that would suggest removal of the capsules, most surgeons would leave them.  Discuss your concerns with your plastic surgeon as s/he knows you best.

Your surgeon is the best person to evaluate you before surgery.  Having said that, if you are still having symptoms from your bronchitis including having a sore throat, it would be best to postpone your surgery.  You want to be in your best health for any surgery.

A tummy tuck will not have any effect on your bladder repair.  A tummy tuck incision will be where your old hysterectomy scar is although it might extend a little longer.  You should be able to have a tummy tuck if you wish.  Discuss your concerns with your plastic surgeon.
