Any surgeon with experience can glance at someone and wing it. Most of the time, you’ll come out great! But the surgeon needs to listen to the patient, not just do what they feel is best. Once explained, most patients understand the rationale and are onboard. A surgeon not willing to discuss their process with a patient doesn’t really know what they’re doing. I’d get another opinion.

As a general rule, I’ve found that if you wear a 34” bra, your base width is around 12.5cm. For a 36” bra, it’s 13.5cm. A 450cc HP implant is around 12.5-13cm depending on the manufacturer. Sure, it would give you cleavage and side boob, but there’s more that goes in to implant selection than that. Most plastic surgeons worth their salt will be able to fix your hernia during the abdominoplasty. If your diastasis is significant, I think you should certainly have them both fixed at the same time because the hernia repair is likely to fail if the diastasis is not addressed. The easiest way for a surgeon to do breast and belly surgery is with general anesthesia. But that is not the only option. Some of us use epidural catheters for tummy tucks and can perform breast surgery very safely and easily using a combination of local anesthetic and IV sedation. We all take measurements differently. Some will measure the entire width of the breast, then subtract according to how much breast tissue you have or how thick your fat layer is. Personally, I measure what I believe to be the width of the pocket the implant will be sitting in (assuming that the implant is under the muscle). You don’t have a “usual” situation, but you may indeed be a good candidate for abdominoplasty. First, make sure that you have optimized your weight loss and are in good health. Then, go for a couple of in-person consultations with reputable plastic surgeons and see what they have to say. In your situation, it really will require an in-person evaluation. Good luck!

When positioned properly, the implant will fill in the skin in the lower portion of the breast and make it look like the nipple is higher. This is with a dual plane technique. A donut lift really doesn’t provide much elevation of the nipple. In your case, I would place the implant through a periareolar incision and then wait 3 months to see if you need the lift. Once it’s done, it can’t be undone – but it can always be done later! Good luck.

That looks like a mole taken off your shoulder by a dermatologist. It’s not a keloid or even hypertrophic. It is wide, but that is likely because it’s on your shoulder. Micro blading is very shallow and should not cause significant scarring. Good luck!

Yes, there was a case in the last year of a teen in Colorado who suffered brain damage as a result of her surgery. I cannot state enough that EVERYTHING seemingly went wrong with that situation. That is EXTREMELY RARE! In my 25 years in surgery I’ve never seen that. We don’t really know what happened, but it should never happen again. In my practice, I discuss anesthesia with every patient. Your scar is called a keloid. It happens unfortunately, but is more common in people with darker skin. It’s not you “fault,” but just your genetics. It happens most commonly where the hair regrows, probably from chronic irritation. I would NOT cut it out again – that often makes it worse. Instead, it should be injected with a steroid to get it to settle down. It often takes a series of injections, spaced about a month apart. Good luck!

The implant can be more narrow than your breast width, just not wider. I disagree with the surgeon stating that it is not important. An implant that is too wide with be forced to wrinkle in order for it to sit in the right place on the chest. This creates folds in the implant. It is well known and accepted that one reason implants fail is due to stress caused by folds in the implant. Therefore, selecting an implant that is more narrow avoids that.
