Should I get a full TT or mini TT with liposuction?
Liposuction +/- Tummy Tuck
Oct 31, 2019
This is a good question. Whether or not a patient needs a tummy tuck depends upon the amount of extra skin in the abdomen. The limitation of Liposuction alone is that the skin must contract over the new contour. If it can’t it just gets wrinkled  and irregular. An experienced Board Certified Plastic Surgeon will be able to evaluate your needs. One option for selected patients to avoid a full tummy tuck is the use of Renuvion J-Plasma tissue tightening at the time of liposuction. It will not …
For subglandular breast implant placement, is silicone or saline better?
Subglandular vs Submuscular implantation with saline
Sep 10, 2019
It appears that the discussion of Subpectoral vs. Subglandular implant placement continues to confuse patients. Simply put, the more soft tissue a patient has to cover the implant, the more natural the look and feel of the result. Going above the muscle essentially leaves out a lot of this desirable tissue thickness and the result can be more artificial (see more of the implant in the upper breast, possible visible wrinkles and with stark demarkation of the implant edge, half a coconut loo…
Can under the muscle be easily exchanged for larger over the muscle implants?
Converting Implants from Subpectoral to Subglandular location
Sep 10, 2019
With rare exceptions, sub pectoral (under the muscle) is far superior to sub glandular for several reasons. First, there is better  coverage of the implant which gives a more natural feel and appearance. Placing any implant, even newer gel, above the muscle risks having the “half a coconut” look with stark demarcation of the implant in the upper pole. With time, most patients upper breast tissue becomes thin and you can get rippling, even in cohesive gel. Also, maintaining the muscle …
If I got a lift, would I need implants? What’s the best incision for my case?
Breast Lift and implant?
Sep 10, 2019
This is a great question. Breast lift with Augmentation is one of the most challenging procedures Plastic Surgeons perform. The need for an implant depends upon your goal for the ultimate size and shape of your breasts. An implant adds volume but also helps maintain the upper pole roundness (vs scalloped out flatness) most patients desire and see as “perkiness”. One stage lift augmentation can be done safely (blood supply is a concern with some techniques) by experienced surgeons but 2 …
Is BWD, implant width and potential impact for BAD, too-high-of-an-implant, or capsular contraction
Breast Animation Deformity
Sep 4, 2019
Great question which brings up 2 issues that are important, but unrelated. Choosing the right implant can be confusing but the most important measurement is the Base width of the patient’s breast as compared to the diameter of  implant. In principle, they should match within a centimeter or so (also the amount and pliability of the breast tissue is very important) to get the best results. This is unrelated to the whether or not there is a considerable animation defect. Post-operative a…
Does getting a lift limit how big the implants can be?
Implant Choice with Breast lift
Aug 23, 2019
This is a great question. The answer is absolutely yes for the following reason. When breasts sag, the nipples become too low and there is too much skin for the volume of the breast. If you simply put in an implant, the nipple and breast slide off the front and it looks awful. Breast Lifts are designed to re-position the nipple to the correct location and remove the extra skin to provide a normal shape. The implant chosen can not be so large as to prevent the necessary skin removal to shape …

Capsulectomy with Breast Lift
Aug 23, 2019
This is a good question but the answer can be a little complicated. Treating your capsule will require either a Neopocket with replacement or Capsulectomy with replacement. The main reason for staging this at the same time as a breast lift is the consideration of blood supply after the Capsule surgery. If you have a fair amount of breast tissue, a single stage might be the best option and safe. If you have little breast tissue with thin skin, it may be too risky to consider a combined p…
Would I get a decent result with a tummy tuck without lipo?
Liposuction with Tummy Tuck
Aug 23, 2019
This is a good question because it concerns several issues that come up a lot during consultations. Body contouring involves using techniques of changing volumes as well as addressing extra skin. Liposuction addresses volume by removing fat, and Tummy tucks are designed to remove the skin. Experienced Plastic Surgeons incorporate multiple techniques and customize each procedure to achieve the best results. Liposuction is usually done of the waist and back to improve the contour and bring…
Do I need a revision to have my breast come closer together and close the gap?
Cleavage after Breast Augmentation
Aug 23, 2019
Thank you for this question because it comes up a lot. The ultimate position and resulting cleavage of the result is usually more related to the patient’s original anatomy than the procedure itself. The closeness or cleavage that results depends upon where the Pectoralis muscle attached to the sternum and this is different in every patient. If the muscles are close together to start, when you place the implant they will be closer together and have more midline cleavage. The opposite is also …
Could a breast lift or implants fix sternum appears to be sticking out of my chest?
Bottoming out after breast lift
Aug 8, 2019
It appears you have bottoming out, or descent of the breast tissue after your breast lift. This is common because the tissue of the breast is weak (which is why you had saggy breasts originally) and, in many patients, does not stay up after the original procedure. Breast lift procedures are imperfect because lifting alone does not change the overall strength of the native tissue. Because of this, there have been many techniques that incorporate breast flap/ internal suspension and/or…
Would a lift fix my droopy breast appearance after a second augmentation?
Double Bubble
Jul 9, 2019
It appears that you had your previous inframammary fold released (commonly done to accommodate a large implant)  and what you see is a ridge where the old fold was with the implant sitting lower on the chest (Double Bubble deformity). The treatment will involve repairing that fold with the implant sitting at the fold instead of below. This may or may not need a product like Stratice depending upon if the fold can be repaired with stitching alone or not. You may need to consider going with a…

Tummy Tuck and risk of DVT
Jul 9, 2019
As long as you are otherwise healthy and standard precautions are taken during and after surgery, you should do great. Most Plastic Surgeons have standard protocols that everyone gets preventative care for DVT but ask your surgeon specifically what they do. Lovenox or Heparin as well as sequential compressive stalkings during surgery is our standard and patients have done great. Best of luck.
I was just recently told I need a breast lift and I want to know if there is anyway around it?
Do I need a lift at the time of my Augmentation?
May 5, 2019
Its very hard to answer that without consultation or at least pictures, but in general if the nipple is above the inframammary fold, you can get away without it. In some patients with the nipple below the fold a lift can be avoided as long as you adjust your expectations for the final result. I have had patients that are willing to accept their nipple below the breast equator pointing down a bit rather than have a lift scar. However, many factors go into that decision and seeking …
Chest exercises after breast augmentation – Risks? What exercises and at what weight would be accep
Pectoralis exercise impact on implant placement
May 5, 2019
The short answer is that , yes it will ruin your implant placement. The long explanation is my impression is that you are one of those workout enthusiasts that doesn’t do anything half way and you also went for a very large implant. This is a double whammy when it comes to results. Working out the Pectoralis Major muscle aggressively after implantation will force the implant down and out laterally on the chest wall and, if your surgeon released your inframammary fold to accommodate the l…
What are anesthesia options for breast lift / mastopexy without amnesia?
What anesthesia is best
Feb 8, 2019
The type of anesthesia used for procedures runs the gamut from local (just the area involved with no feeling), to various types of sedation (patient sleepy or gently asleep but protective reflexes intact)  and finally a full general (totally asleep). Which type is used reflects the duration, complexity and magnitude of the procedure and most importantly the safety . Remember the goal is to make the experience as pleasant as possible with the most safety.  It is common for patients to over…
