I would recommend getting a few consults with board-certified plastic surgeons. In general, we try to limit combined surgical procedures to 6 hours. You may need to have separate procedures in order to safely address all of your areas of concern. I would recommend prioritizing your areas of concern and addressing the top 2-3 areas that bother you the most.

I an unaware of a reason to come off Tramadol, but I would refer you to your surgeon and ask him or her the reason for their decision. More importantly is the issue of your cold. I would recommend being in close contact with your surgeon and their anesthesia team in order to determine whether it is safe to proceed with your surgery.

I believe that it can be helpful to you to see a few surgeons in consultation and start planning your journey. I would advise reconsulting with your surgeon if you do loose a substantial amount of weight as the surgical plan may change.

In general, I tell my patients that 50% of the swelling is gone at 6 weeks and that it takes 6 months for the majority of swelling to resolve. Be patient with your recovery. Swelling is an expected part of the healing process. Compression garments can help expedite your recovery.

For most of my patients, I allow light cardio 3 weeks after surgery (light job, elliptical without arms, stationary bike). All activity without restrictions can resume at 6 weeks. However, I recommend consulting with your surgeon in case your surgery was more complicated and requires longer recovery.

I would recommend seeing your surgeon. Scars that are painful or itch can be a sign of hypertrophic scaring where a thicker scar develops. Early treatment gives the best result. Therefore, see your surgeon for an evaluation. He or she may consider silicone sheeting or steroid shots into the scar itself to help soften the tissue.
