Hello.
Yes, it is true thin-skinned noses have a higher difficulty for rhinoplasties because underneath structures can show sharp edges, and there is an increased risk for skin necrosis. Avoiding the creation of sharp surfaces, making them curved and soft, and adding tissue as fascia in between structures and skin is what is used to reduce this risk. Hyperbaric oxygen therapy after surgery for 5 consecutive days can be used to prevent skin necrosis.

Hello Scarlet.
When a nose is narrowed with a rhinoplasty, the whole face looks slimer after. Try to look before and after pictures on patients with wide noses and you will see this effect. I do not believe you need a blepharoplasty or a buccal fat pad removal. Buccal fat removal is useful on round faced patients and this is not your case, you have a nice oval shape.

I do agree with your perception about your nose size, being slightly bigger than your ideal size. With a rhinoplasty, your tip structure will become strong enough to avoid dropping while smiling, make it narrower on front views and make your bridge reduced in height. Computer morphing is an especially useful tool so you can have an idea of an approximate surgical result.
