Yes, it appears you have fairly mild tuberous breast deformity. Depending on breast shape goals / volume, likely breast augmentation alone should suffice. You should anticipate some worsening of the nipple / areola shape immediately after surgery, but will likely settle out well over time. If the "poochiness" of the nipple bothered you down the road, it could be corrected but would require additional incisions around the areola.

Given age and photos of your skin, likely liposuction alone will suffice. If there ends up being a little skin redundancy in the lower part of your tummy, some skin removal could be contemplated as an additional surgery/cost. But if future pregnancies are likely, I would suggest just waiting on that.

I agree with many of the others. An implant will give you the round shape and upper pole fullness, but you will need to remove or reposition the breast tissue that is sliding off the chest. Therefore, implant with some version of a breast lift or reduction (depends also on the specific surgery you had in the past).
Galaflex is not likely needed. Save the money and complication risks. My two cents.
