You are quite young to have aging changes in the face but the hollows in your chin and lower face mimic those that we would usually see in someone older. I would recommend treatment with hyaluronic acid filler to both your chin to softly augment. Such fillers are reversible if you do not like and can last as long as two years, without the risks of surgery. Less filler may be needed after follow up treatments due to lasting newly formed skin collagen.

Your picture is hiding the lower part of your temples, so it is difficult to see the full benefit you would get from filler there. Filler in the temples is helpful to neutralize hollows, and yes, it can lift the lateral brow and upper lateral cheek a touch in some people. I would not recommend filler to temples to improve overlying skin texture problems like you describe - visible pores, thinness. The Yag or Fractionated 1550nm or Fractionated 1540nm lasers, can be used to tighten lax neck skin, but usually requires repeat treatments, probably 2 or 3 treatments to approach the efficacy of the Thermitight (subcutaneous RF). The nice thing about the Yag laser treatment is it does not usually require anesthesia, and there is no significant downtime. The fractionated 1550nm and 1540nm have a bite to them, so require at least topical anesthesia and sometimes oral light...

In addition to doing filler in the marionette lines, and filler in the hollows of your chin, I would suggest filler to the cheeks and temples which can help to lift when there is skin laxity in the jowls over the DAO. Another point, if you have been using Botox for awhile, and not getting a good response with the typical dose (1-3 units only per side to DAO) it is possible you are immune to it, and could try Dysport or Xeomin.

I can see in your photo what looks like a small pouch of lax neck skin presumably at the site of the prior seroma. I do not see typical scar tissue which might appear thickened. ThermiRF is helpful for skin laxity in the neck as you seem to have and seems to be a good first treatment choice. Hopefully, it will tighten that lax appearing skin enough to obviate the need to excise the scar. Good Luck

I perform Themitight and agree with your surgeon's estimates. You would likely get just modest (50% ish) improvement with Thermi. However, many people are happy with modest improvement.
