Hello and thank you for your question. I am not a big supporter of the donut mastopexy.
In theory, because of the single scar around the areola, it seems like an ideal procedure due to less scarring. The reality is that many patients are unhappy with their results; either because the areola complex stretches out over time resulting in excessively large areolas or that not enough skin was removed resulting in a less than ideal lift.
Could a breast lift or implants fix sternum appears to be sticking out of my chest?

Thank you for your question and photos. In order to restore volume to your breast, your best option would be breast implants. This can be done safely after breast reduction surgery.

Thank you for your question. By looking at your photos, your best option would be a full tummy tuck.
Liposuction would not be the best option as most of your fat is intra abdominal (visceral) and this fat is not amenable to liposuction.

Re: TT I recommend that you wait at least one year from the reversal of your ileostomy before considering a TT. Depending on how much skin can be removed from the lower abdomen, it is possible to have the scar of the ileostomy site removed.
If you have a midline abdominal scar, this scar too can be revised at the same time as well.

By looking at your photos, I would recommend a breast lift with reduction and using a larger sized implant so that your breasts will be shaped by the implants. I also recommend a full abdominoplasty with muscle repair.

Ideally I like to remove my drains by day 14. However, when the output is higher than 30cc/day I consider leaving the in drain longer. I would suggest a breast reduction along with a tummy tuck and upper lateral thighs. These procedures can be performed at the same time. I would suggest waiting to have anything done to your arms. In my experience, liposuction of the upper arms following massive weight loss, may result in saggy skin.
