Usually I request a minimum of 2 weeks prior to the procedure. It takes some time for the effects of smoking to reverse. Once you stop just stay a non-smoker. There is no need to fix your face just so you can get lung cancer.

As a fellowship-trained Mohs surgeon I rarely give oral antibiotics for standard cases. There are however times when it is necessary. It was likely prescribed by the doctor that closed the wound and you should call him/her to see if it is necessary.

Mohs micrographic surgery is not your only option but it is probably your best. With a cure rate over 98% and being tissue sparing it will give you the smallest defect and usually scar. Excision, electrodessication and curretage, are some other options but have lower cure rates.

If you get too much granulation tissue it can make a full thickness skin graft not match up well. Were you offered other closure options? A defect like yours would take a paramedian forehead flap really well. Consult with your surgeon, only he/she can determine if the granulation is appropriate for grafting.

It is not normal to have bleeding one week after surgery. The separation is probably the problem but you should call your physician so the wound can be evaluated for a hematoma or infection.

Occasionally when we do Mohs some of the superficial nerves are damaged in the process. The damaged nerve can give you a numb sensation or headaches which can last beyond 6 months. There are better medications for nerve induced pain. Go back to your Mohs surgeon and try another medication.
