Transax subfascial is a very difficult dissection but not impossible, it will def give you the augmentation you want but you are entirely relying on the implant to provide a minimal amount of lift. I would confer with your surgeon about the need for a lift and the pros and cons

I think transaxillary is a great route, and for primary augmentation unless otherwise indicated I almost exclusively use the transxillary route for this purpose. I think being comfortable with your surgeon and seeing his results helps to select the proper surgeon
