It is hard to fully assess without examining in person, but best I can tell, you have more of a fullness and protrusion above rather than significant hollows. Fillers can help as well, and you may find 50% of doctors may recommend surgery and 50% may recommend fillers, but I would think blepharoplasty may be very effective and longer lasting in result.

It appears a significant contributor to your heavy lids and forehead ache is the brow position. your eyelids have already been addressed and likely there isnt much redundant skin remaining but more just the low set brow weighing down on the eyelid. A brow lift will most likely give you the biggest improvement as well as botox to the glabella between the eyebrows.

This is a challenging one to answer. Most likely your brow is higher on that side because the lid is droopier on that side and your brain is compensating by raising the eyebrow hoping to get the eyelid more open. Ideally surgery for your ptosis will reduce this drive to raise your eyebrow on that side and it will get to a level closer to the other brow. This isn’t always the case however and there is no great way to predict the final result until surgery is tried. Hope that helps!

Thank you for reaching to our community! There can be options to help with your droopy upper lid and to adjust the lower eyelid position to reduce the appearance of one eye looking bigger than the other (when the lower eyelid has extra white showing above it compared to the other, it gives that illusion). How to address that difference is the question that is hard to answer without evaluating you in person and getting a feel for the eyelid tension and mobility.

Thank you for great pictures to evaluate your eyelids and give you an appropriate answer. There are essentially two ways to reduce lower eyelid bags. One is with filler to fill the hollowing below the fullness of the eyelid itself, smoothening the transition between the lid and the cheek. Restylane is the most common filler used in this area.
Lower eyelid blepharoplasty can help as well as it removes some of the excess fat above and actually repositions some to fill in the hollowing below….

There are many considerations when deciding what can be done for lower eyelid surgery. It appears that if the outer corner is rounded, revisional surgery can help sharpen the corners, but this may need to be done in conjunction with a graft on the inside of the eyelid called a spacer graft (if the lower lid is retracted down). I don’t see any contraindication in administering Botox for the “11” lines between the eyebrows as it is away from the surgical site. I would however wait 1-3 months to do any Botox to the smile lines around the eyes as that is in the area of a lower lid surgery. Hope that helps!
