If you are having an abdominoplasty (Tummy tuck) I usually recommend two weeks off of work, if it is just breast surgery I usually recommend 1 week off and 5-6 weeks from strenuous physical activity for both surgeries. Some patients might be able to make it back a little sooner but that is the exception more than the rule. You always want to wait until you have enough time down to recover and bounce back after surgery.

It looks like you have done a great job with weight loss after your bariatric surgery. I would recommend that if you have a hernia, that you get it repaired by a general surgeon at the time of the lower body lift or before. A hernia repair can increase the risk of loss of your belly button if not done with the plan of an abdominoplasty in mind. Your preop testing plan should be based on a thorough preop history and physical. Many young healthy patients do not need additional preop labs unless they have a significant medical issues or a pertinent family history of issues with anesthesia etc. In the old days they ordered labs on everyone, but the yield of valuable information ended up being so low that they no longer do that for many minor surgical cases.

I would recommend that you talk with another physician who does a lot of inferior pedicle breast reductions. I have done many breasts with long pedicle lengths that did just fine and had no nipple necrosis. Some people have been trained that you always need to do nipple graft and that is not the case for many patients.

Theoretically, you could do both procedures at one time but it would significantly increase the risk of complications, because of competing areas of tension and decreased blood supply. Therefore, I would never recommend doing both at the same time. This falls under the category of just because it can be done does not mean it is a good idea.
