Shortness of breath long term following breast augmentation is almost unheard of. First, I would recommend that you see your primary care doctor for a complete evaluation for that complaint. Regarding your cosmetic result, You may benefit from a pocket revision to improve the position of the implants.

The best way to estimate your resulting size is to visualize a 650cc implant. You can create this with a ziploc bag and 2 3/4 cups rice. This is the volume that will be removed from your current breast size.

Without photos or an examination, it is difficult to give you a concrete opinion. However, I would recommend removal first without a lift, as it is always possible to do the lift secondarily. I have seen many cases where the tissues retract nicely and make the lift unnecessary. The fact that you were an A cup and do not have stretch marks, make this a likely outcome.

In my practice, I advise no exertion of any kind for 2 weeks after surgery. Beyond that, activity levels are dictated by your pain tolerance. Three weeks should be enough time for your recovery, barring any complications.

With smooth, intact implants, the issue of capsulectomy with BII symptoms has no clear answer. In my practice, I make that determination at the time of surgery. If the capsule is extremely thin and membranous, there is no benefit in its removal. If, on the other hand, it is thick, fibrous, or calcified, then a complete capsulectomy should be performed. The removal of the posterior capsule, (which usually distinguishes partial vs. complete) is sometimes easily...
