Your pictures show about as much improvement as the procedure can offer. What remains, as far as the pictures show, is deeper visceral fat which is unreachable, plus possibly lax abdominal muscles, both of which leave you with less than a “washboard abdomen.” A surgeon experienced in tummy tuck may be able to help. All in all it appears you achieved as much improvement as is reasonable. The type of anesthesia used is irrelevant. 
I would like to improve the appearance of my jaw and neck, would Kybella work?

Although you’d improve with non-invasive, incremental procedures such as Kybella, the expense and the wait time until you achieve your final result are hard to swallow compared to the minimally-invasive, local anesthesia SmartLipo. It takes 20 minutes and you results are visible within weeks and continue to improve over several months. With Kybella you won’t even be finished your treatment sessions for 3-5 months, and then have to wait another several months for the final result. 
Can I massage my chin to drain the fluid from my Kybella injections? 4 days post op.

The swelling you see and feel is not just a pocket of fluid waiting to be massaged away. There is considerable inflammation And tenderness which likely won’t be helped along by manipulation, so best to just let the process work itself out. It won’t take very long. The inflammation will leave collagen in its wake with its resultant tightening. 
7 weeks after Smart Lipo. Have you seen this before?
Accurate delays healing

Your healing is certainly delayed more than the norm. Accutane is known to delay skin healing. In dermatology procedures such as chemical peels and laser resurfacing are not done unless at least 6months have passed after Accutane. The places will heal, but it’s still going to take a few more weeks. 
After Smart Lipo, my white blood cell count is high

Your elevated WBC count is a general measurement. The differential count is important in determining a cause, but I would not look to your Lipo procedure as the likely culprit. The fistula is more likely to be responsible, or some other mild occult infection. Just get it resolved before any surgery. 
Is it possible to get smart lipo with a BBL instead of the traditional lipo?

Fat transfer depends on living cells to survive their being moved to a new location. By definition the SmartLipo laser kills the cells right off the bat, so they aren’t useful at all in BBL.

The Sculpsure laser only works when there is good contact between the skin and applicators. The amount of fat squeezed within is not a factor, but the settings used certainly are. If you felt little or no heat when the machine was cycling, the settings were likely too low. If it was tolerable but uncomfortable your settings were ok.
