Typically yes. Most surgeons will combine fat removal with gland removal to achieve a harmonious result. This can be either direct excision or with liposuction. A smooth contour throughout the entire breast is always the overall goal.

I typically don’t recommend crescent lifts since the risk/reward ratio isn’t favorable. The lifting benefit is minimal and the possibility of distortion is greater. If anything, I recommend a circulareola lift more which adds a little more scar but much more lifting ability.

Breasts are a different story than the rest of your anatomy. The nerve anatomy is different on every person. Although you have a dominant nerve for nipple sensation, usually there are multiple that innervate to help reduce the chances of complete nipple sensory loss.

This depends on your goals. If you want to be functional, most patients go with a more conservative size. If that means a lift, then patients tend to choose that. I have 3D Vectra imaging software in my office to facilitate these discussions so you may want to ask if they have something like that to help.

There are a lot of actual stitches but the actual number doesn’t matter. What matters is the type of stitches. I use 2 long permanent stitches and then reinforce it with alot of interrupted absorbable sutures. This is a very common and long-standing technique.
Does dual plane implant placement always require cut to the muscle?

Technically dual plane means you dissect under the gland and the muscle. Therefore it is does require release of the muscle slip onto the rib periosteum. It is a great technique especially for patients with ptosis.
Should I have fat transfer done at same time as breast implant removal and lift or wait?

Thank you for your question. Fat Transfer is normally performed under general anesthesia due to the liposuction procedure needed in order to harvest the fat for the transfer. Fat Transfer can be performed at the same time as an implant removal, however, it may be recommended to stage your surgeries. Consult with a board certified plastic surgeon who has experience with fat transfer to the breasts, and communicate your post-op expectations post implant removal. Good luck on your journey!

It would be best advised to consult with your surgeon to discuss scar massage techniques or the usage of Biocorneum or Vitamin E, which may be recommended but not always necessary this early on. Thank you for your question. It is difficult to answer this without an examination or photos. However, typically if you have excess skin and/or lax muscles, you can be a candidate for an abdominoplasty. Of course, you have to be fairly healthy and at a stable and low weight. Most plastic surgeons prefer patients BMI’s to be lower than 35. I hope this helps and remember to seek a board certified plastic surgeon.

Exparel is a long lasting anesthetic that is typically injected when you are asleep during surgery. I inject into the muscle fascia during abdominoplasties and the incision sites. It usually doesn’t contain epinephrine. It has definitely made a difference postoperatively for my patients.

I have my patients begin scar massage at 3 weeks postop daily for about a year. Scar creams such as Biocorneum or Vitamin E is recommended but not always necessary. Be sure to use sun screen in the scars even when clothed in intense sunlight.
Does a tummy tuck actually remove fat cells, yielding the same result as lipo?

An abdominoplasty removes fat cells and skin while liposuction just removes fat cells. Be sure to know that not all fat cells are removed so those left behind can hypertrophy (get bigger) if the patient gains weight after surgery. Thanks for your question.
