After reviewing your photos I think you would benefit from liposuction of medial thighs, flanks and the lower, small of your back. I could see you getting nice results from a low volume fat grafting to hip dips to improve your overall shape however I do not feel you will get the projection you are aiming for since you only have a small amount of fat available, but it will provide a significant difference since you have a small frame.
Risks of using leg fat for fat transfer to breast procedure?
Lumpiness should not be a problem if done carefully.

We often transfer fat to the breasts, and it works very well. We use fine fat particles and use a centrifuge to concentrate the fat.
You should make sure you have a baseline mammogram before surgery because it will be different afterwards.

You don't have a lot of extra fat. It seems that you have a little on you abdomen and flanks. Not much on your back. You can also harvest some fat from your inner thighs.
Even though it is not a lot of fat, because you are thin, it will make a big difference to the shape of your buttocks.

It sounds from the description that the mesh is a small piece and even if this is thickened and scarred it can be removed fairly easily and the abdominal muscles imbricated or folded in to tighten them.
Am I a good candidate for forehead reduction?
The final decision will need to be made in surgery.

I would aim to remove about 1.5 to 2cm of the forehead skin. The actual amount will depend on how mobile you scalp is after lifting it up. It sometimes takes a lot of work to stretch the scalp and move it forward.
I think this procedure will help you a lot. You seem to be an ideal candidate.

The implants can certainly be drained with a needle with local anesthesia in advance of the surgery, and I have done this several times. This should not affect the capsule removal, and should make complete capsule removal easier.

There are many advantages to a Drainless Tummy tuck.

I have been using a progressive tension closure with no drains for several years now. I only wish I had done this earlier. The healing is better with less tension on the suture line, the dead-space under the skin flap is drastically reduced, the incidence of seromas is less, even than when I used drains.
In addition the discomfort and inconvenience of the drains is eliminated.
It does take a bit longer, but the benefits make it worth while.

As long as you are comfortable with your weight and not planning any drastic change I think it is fine to do a BBL.
Thinner patients have a better response with less fat volume. (I.e a little makes a big difference)
I do not restrict my BBL procedures to any particular BMI. But iff someone is very overweight and planning to lose weight I would delay.

I would avoid the thigh lift if possible.

I think liposuction may work well instead of a thigh lift. You can use the fat for the BBL.
Your surgeon may have had a problem with the length of the procedure or positioning in surgery.

I reviewed the photographs that are available online.  I do not use the modality. It seems to me that your abdomen looks particularly good for someone who has had several children. There is almost certainly some stretching of the muscle wall which is unlikely to be corrected by stimulating the muscle to contract. 
For a woman that weighs 147 lbs and is 5 ft 1, what size implant should I consider?
There is no absolutely correct answer as to the size of the implants.

The size of the implants is limited to some extent by your body type, but to a large extent it is up to you to decide what size breast you would like to have. I do this in my office by using sizing implants that are tried on in a bra. With this approach we can tell very accurately what size the patient likes and what is too much. This allows for an accurate selection of the correct implant.
I developed fat necrosis of the face after a fat transfer. How safe is Renuva or matrix to remodel
I recommend that you get another opinion from a qualified plastic surgeon.

Fat necrosis in your face should be extremely rare. I am concerned about the technique that was used for your fat grafting. I would not like you to use a processed fat product that has no live cells in it. It would be better to use your own fat again done in a proper way to fix the problem.
